By submitting the below form, you acknowledge the following:

• A home visit may be required. This may be in person, via video, or zoom/FaceTime.

• An adoption fee is non-refundable, should I adopt an animal.

• The animal I am applying for will be adopted solely as a pet — breeding, selling, etc. is prohibited.
